At Optimum Care Partners, we help patients transition from hospital-based care to the next stage of their healthcare journey. Our transitional and home visit services are designed to support eligible patients after hospitalization, particularly those who may benefit from additional medical attention and care coordination at home.

High-Risk Patient Identification and Screening

Our care coordination process may begin during or after a hospital stay. Patients who may benefit from transitional or home-based care can be identified based on their medical history, current health needs, and post-hospitalization circumstances. When appropriate, a referral is made to our team for further review and assessment.

Our care team evaluates each patient’s needs and, with the patient’s consent, coordinates an appropriate home visit when home-based care is suitable.

At-Home Visits for Personalized Care

Home visits allow our healthcare team to evaluate patients in a familiar environment and address healthcare needs following hospitalization. During a visit, our team can review the patient’s current condition, medications, care plan, and other factors that may affect their recovery.

We take the time to understand each patient’s circumstances and provide guidance based on their individual healthcare needs.

Coordinated, Collaborative Care

At Optimum Care Partners, we work to coordinate care among appropriate healthcare professionals and services involved in a patient’s recovery. When appropriate, this may include communication with specialists, home health providers, hospice services, caregivers, and other members of the patient’s care team.

This collaborative approach helps support continuity of care and keeps the patient’s individual needs at the center of the care plan.
