Need to refer a patient to Optimum Care Infusion Center? Our team is here to help! You can use our referral forms or submit your own along with any additional documentation (patient records, case notes, etc). We’ll take it from there with getting insurance approval and will work with your office if any additional documentation is needed.
We work hard to create the best possible patient experience! From our helpful and welcoming staff to our comfortable infusion center with audio/video streaming, heated massage chairs, and well-stocked snack bars, we focus on maximizing patient comfort during infusions.
