It is essential to receive your annual flu vaccine each year to help prevent an illness that could lead to unnecessary medical visits and hospitalizations. The drive through flu clinic is a great, convenient option! No worrying with getting out of the car, fighting with car seats or waiting in the waiting room!
How do I get a flu shot?
There are three easy steps to receive a flu shot at Optimum Care Partners:
- Fill out the form below or show up day of with ID and Insurance Card ready (please have these available even if you fill out the form)
- Drive up to our clinic between 9am and Noon on October 4th, 2025
- Receive your Flu Vaccine!
